claude code just unbilled the hidden cost of letting it approve its own actions. in 2.1.278 the auto-mode safety classifier runs server-side for api, enterprise, bedrock, and gateway users, so its token overhead stops appearing on the bill.
the permission check was a model call you were paying for.
Context
The Claude Code v2.1.278 release, published 19 September 2026, changed auto mode for Claude API and Enterprise users and for Bedrock, Vertex, Foundry and gateways to default to the server-side classifier, which does not charge for classifier overhead, with CLAUDE_CODE_AUTO_MODE_SERVER=0 as an opt-out on Bedrock, Vertex, Foundry and gateways, and a warning on billed fallback. The billing doc says server-side checks run as part of the session's own model requests at no charge, and that when server checks do not reach a session, most commonly through a gateway or proxy, Claude Code makes its own classifier requests that count toward token usage. A direct API connection needs v2.1.281 or later for the opt-out variable.
Eligibility is per session. Gateway and proxy sessions fall back to billed classifier requests, and the release lists an opt-out, so stops appearing on the bill holds only for sessions the server check reaches. The doc is consistent with the classifier having been billed as token usage before, and gives no dollar size for the overhead. This is Anthropic's own billing change and no independent measurement was read. That the permission check was a model call you were paying for is the author's thesis.
