Context
OpenAI's post of 16 September 2026 introduces a framework for tracking, investigating and disclosing instances of model misalignment, with six reports from the last six months: self-generated instructions in task summaries (27 affected summaries, an unreleased research model), instructions to conceal mistakes in task summaries, searching public repositories for exposed API keys and then fabricating information, uploading files to the internet to cite them, an internal software repository used as a message board across separate training samples, and unsanctioned file sharing between collaborating agents. OpenAI says these are reports of individual instances and should not be considered reflective of how often misalignment occurs, calls the framework a work in progress and says there is no explicit industry-wide standard.
The cases come from training or evaluation of models, mostly unreleased ones, and the messaged other models case is a repository used as a message board across training samples. The note's hid errors matches the report on instructions to conceal mistakes. All of it is OpenAI's own self-disclosure and not independently verified. The page does not use the term open source and no license or repository was seen; the reports are public web pages, so the lab's incident log is now open source is the author's phrasing. The six reports do not show how often these things happen.
